{"id":11641,"date":"2023-05-17T09:09:36","date_gmt":"2023-05-17T07:09:36","guid":{"rendered":"https:\/\/pharmanutra.it\/iron-and-celiac-disease\/"},"modified":"2023-05-17T09:09:36","modified_gmt":"2023-05-17T07:09:36","slug":"iron-and-celiac-disease","status":"publish","type":"post","link":"https:\/\/www.pharmanutra.it\/en\/iron-and-celiac-disease\/","title":{"rendered":"Iron and celiac disease"},"content":{"rendered":"<h2>Introduction<\/h2>\n<p><strong>Celiac<\/strong> <strong>disease<\/strong> is a chronic autoimmune disease that affects the small intestine in response to the ingestion of <strong>gluten<\/strong>, a protein found in wheat, barley and rye and affects about 1% of the population.<\/p>\n<p>In people with celiac disease, the immune system mistakenly attacks the lining of the small intestine when gluten-containing foods are ingested, which can lead to damage and <strong>inflammation of the intestinal wall<\/strong> (Figure 1). This mechanism causes the alteration of the intestinal mucosa, characterized by atrophy of the villi (flattening) with excessive accumulation of pro-inflammatory cells, thus limiting the normal function of nutrient absorption in the intestinal tract.<\/p>\n<figure id=\"attachment_6450\" aria-describedby=\"caption-attachment-6450\" style=\"width: 416px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-6450 size-full\" src=\"https:\/\/www.pharmanutra.it\/wp-content\/uploads\/2023\/05\/iron-and-celiac-ENG.jpg\" alt=\"\" width=\"416\" height=\"228\" \/><figcaption id=\"caption-attachment-6450\" class=\"wp-caption-text\">Fig.1 Representation of normal and celiac Intestinal Villi<\/figcaption><\/figure>\n<p>Symptoms of celiac disease can vary widely from person to person and may include abdominal pain, bloating, diarrhea, constipation, fatigue, anemia, weight loss, and rash. Celiac disease, if not treated, can lead to complications such as malnutrition, osteoporosis, and an increased risk of certain types of cancer.<sup>1<\/sup><\/p>\n<p>The only treatment for celiac disease is a strict, lifelong <em>gluten-free<\/em> diet; this involves eliminating all foods and products that contain this protein, including bread, pasta, cereals and many processed foods. In some cases, supplements or medications may also be recommended to help manage symptoms or address nutritional deficiencies.<\/p>\n<p>The diagnosis of celiac disease is made through a blood sample to search for specific antibodies to gluten and then a biopsy of the small intestine to look for signs or damage to the intestinal wall, typical of celiac disease. <sup>2<\/sup><\/p>\n<p>Iron deficiency anemia (IDA) is a common complication in celiac patients. National and international guidelines recommend screening for the evaluation of iron parameters in this category of subjects, such as <strong>hemoglobin<\/strong> and specific parameters of iron metabolism that indicate the amount of iron present in the body (<strong>serum iron<\/strong> and <strong>ferritinemia<\/strong>).<\/p>\n<h2>Iron deficiency anemia<\/h2>\n<p>According to the World Health Organization (WHO) the physiological range of <strong>hemoglobin<\/strong> (Hb) varies according to gender and age: men 14-18 g\/dL, women 12-16 g\/dL and children 10.5-13.5 g\/dL.<\/p>\n<p>When the Hb value is lower than the above values, we speak of iron deficiency anemia, which is also confirmed by the measurement of other important parameters:<\/p>\n<ul>\n<li><strong>Ferritin<\/strong>, storage protein capable of containing up to 4500 iron atoms (physiological range: 30-300 ng \/ mL);<\/li>\n<li><strong>Sideremia, <\/strong>which indicates the amount of circulating iron bound to a specific protein, called transferrin (physiological range: 50-180 mcg\/dL)<\/li>\n<li><strong>Transferrin Saturation<\/strong> (%TSAT), a protein that transports iron in the tissues and districts responsible for its use. (physiological value: &gt;20%)<\/li>\n<\/ul>\n<p>When it comes to <strong><em>sideropenia<\/em><\/strong>, however, there are not always evident hematological alterations; in fact, hemoglobin levels may be normal, but ferritin and transferrin saturation are low.\u00a0 In this case it is strongly recommended to increase the amount of iron through diet or food supplementation.<\/p>\n<p>The incidence of iron deficiency is found in 10-15% of subjects suffering from celiac disease.<sup>4<\/sup><\/p>\n<h2>Iron and celiac disease supplementation<\/h2>\n<p>Iron supplementation is often required in patients with celiac disease, who often have poor iron absorption due to intestinal inflammation<sup>3<\/sup>. Oral iron formulations are often preferred over intravenous formulations, due to the greater convenience of use and patient preferences. However, supplementation of the standard oral iron formulations in celiac patients may aggravate some symptoms of the disorder, such as abdominal pain, bloating, diarrhea and constipation. <sup>5<\/sup><\/p>\n<h2>Sucrosomial\u00ae iron \u2013 clinical evidence<\/h2>\n<p>To counteract the states of iron deficiency, PharmaNutra Spa has developed <strong>Sucrosomial\u00ae Iron<\/strong>, marketed under the name of <strong>Sideral\u00ae<\/strong>.<\/p>\n<p>This new technology represents an innovative oral formulation containing iron, in which iron pyrophosphate is wrapped in a matrix of phospholipids and sucrose esters of fatty acids; this structure is called Sucrosome\u00ae.<sup>4<\/sup> (Figure 2)<\/p>\n<div class=\"mceTemp\"><\/div>\n<figure id=\"attachment_6451\" aria-describedby=\"caption-attachment-6451\" style=\"width: 300px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-6451 size-medium\" src=\"https:\/\/www.pharmanutra.it\/wp-content\/uploads\/2023\/05\/sucrosoma-eng-300x150.jpg\" alt=\"\" width=\"300\" height=\"150\" srcset=\"https:\/\/www.pharmanutra.it\/wp-content\/uploads\/2023\/05\/sucrosoma-eng-300x150.jpg 300w, https:\/\/www.pharmanutra.it\/wp-content\/uploads\/2023\/05\/sucrosoma-eng-768x384.jpg 768w, https:\/\/www.pharmanutra.it\/wp-content\/uploads\/2023\/05\/sucrosoma-eng.jpg 900w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><figcaption id=\"caption-attachment-6451\" class=\"wp-caption-text\">Fig.2 Structure of the Sucrosome\u00ae containing iron pyrophosphate.<\/figcaption><\/figure>\n<p>The result of this technology leads to a high gastrointestinal tolerability and easier absorption of iron in the intestine. In recent years many scientific evidences (both preclinical and clinical) have been published in international scientific journals confirming the effectiveness of Sucrosomial\u00ae technology.<\/p>\n<p>Gastroresistance is given by the sucrester matrix, as demonstrated in <em>in vitro studies, <\/em>which protects iron from the acidity of the pH of the stomach.<\/p>\n<p>This allows the Sucrosome\u00ae to arrive intact in the intestinal mucosa, where it is absorbed by enterocytes as a vesicular structure, therefore a completely different mode of absorption compared to conventional iron which is instead absorbed by a specific membrane channel placed on the apical part of the enterocytes, called Divalent Metal Transporter type 1 (DMT1), responsible for the reduced absorption of iron and,\u00a0 consequently, of its common adverse effects (e.g. gastralgia). <sup>5<\/sup><\/p>\n<p>&nbsp;<\/p>\n<p>In a clinical study conducted by Dr. Elli<sup>6<\/sup>, \u00a043 subjects with celiac disease were divided into two groups: the first group consisted of 24 subjects, who had reported previous side effects following the use of iron sulfate, were supplemented with<strong>\u00a0 30 mg of SiderAL\u00ae<\/strong> for 90 days; while, the second group (control arm) consisted of 19 subjects, who were tolerant to <em>iron sulfate<\/em>, continued the same treatment, at a dosage of 105 mg per day, always for 90 days.<\/p>\n<p>The results showed that treatment with SiderAL\u00ae allowed to increase haemoglobin values by administering a <strong>dosage 3 times lower than<\/strong> <strong>iron sulfate<\/strong>.<\/p>\n<p>In addition, subjects taking Sucrosomial\u00ae iron experienced <strong>less severe<\/strong> gastrointestinal symptoms and improved <strong>overall well-being <\/strong>and, in contrast to the control group treated with iron sulfate, where there was greater severity of gastrointestinal symptoms during the study period.<\/p>\n<p>In conclusion, Sucrosomial\u00ae iron is <strong>better tolerated and suitable<\/strong> for patients suffering from celiac disease and iron deficiency anemia, since its innovative formulation allows good intestinal absorption although the intestinal mucosa is inflamed<sup>6<\/sup>.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter wp-image-6452 size-full\" src=\"https:\/\/www.pharmanutra.it\/wp-content\/uploads\/2023\/05\/sideral.png\" alt=\"\" width=\"600\" height=\"344\" \/><\/p>\n<p><em>Bibliography<\/em>:<\/p>\n<ol>\n<li>Mahadev S., et al.
